Join the practice
The doctors welcome new patients who live within our practice area. It is necessary to complete an application form, it is also helpful if you are able to hand in your medical card.
All appropriate enquiries and examinations will be undertaken as a result of a consultation. Patients are encouraged to consult with their ‘usual’ doctor whenever possible in order to maintain continuity of care.
Medical treatment is available from the date of registration. Please contact reception for further information.
If you would like to register with the practice please print and complete the registration form and the health questionnaire. Once completed please bring both forms to the practice with proof of your address and photo ID.
Live within boundary area?
To be eligible to register at the practice, you must live within the practice catchment area.
Non English Speakers
